Pearls are optional run modifiers in Pass the Fear that trade comfort for power. When you unlock higher campaign difficulties — Nightmare, Hell, Reality, and Blood Moon — the lobby lets you select a number of Pearls before launching. Each Pearl grants a strong bonus and attaches a meaningful penalty. Stack too many without a plan and even god-tier relic lines collapse under the combined drawbacks.

When Pearls appear

Pearls are not part of your first Normal or Hard learning runs. They enter the flow once you unlock high-difficulty tiers — see difficulty guide for the Hard → Nightmare / Hell / Reality shortcut added in patch 7.26.

Typical Pearl selection happens in the pre-run lobby:

  1. Pick campaign difficulty (Reality, Blood Moon, etc.).
  2. Choose how many Pearl slots the tier allows.
  3. Assign specific Pearl effects — each with upside and downside text.
  4. Queue solo or co-op; the whole party shares Pearl rules in multiplayer.

Treat Pearl selection like Chaos Battle Scar stacking: add one new Pearl per successful clear until you understand its failure mode.

Power vs penalty design

Pearls follow the same design philosophy as tarot and scars — readable trade-offs, not hidden gotchas. Common pattern categories include:

  • Economy Pearls — richer shops or drop rates paired with reduced healing or tighter inventory.
  • Combat Pearls — raw damage or proc bonuses paired with Silence-like skill restrictions or fragility.
  • Build Pearls — extra tarot or relic flexibility paired with slot limits elsewhere.
  • Timer Pearls — Storm Wall or mission clock adjustments paired with elite density spikes.

Because Pearls stack with difficulty tier modifiers and optional Chaos scars, multiply penalties mentally before clicking accept. A Pearl that nerfs healing hurts twice on Blood Moon where chip damage already spikes.

Patch 7.27 Pearl balance

Patch 7.27 (July 27, 2026) rebalanced several Pearl-adjacent systems after Blood Moon launch feedback:

  • Tarot Card slot constraints relaxed — Pearl loadouts that previously blocked certain tarot combinations now allow more flexible five-card planning.
  • Destroyed relic and treasure slot rules eased — fewer hard locks when Pearls interact with inventory pressure.
  • Healing trigger limits — caps on how often healing can proc prevent infinite sustain loops that Pearls + economy relics could enable.
  • Item size adjustments — inventory tetris under Pearl penalties feels less punishing on long Reality routes.

If you skipped Pearls during launch week because tarot slots felt too tight, retest after 7.27 with a relic synergy plan that leaves one flex slot open.

First Pearl pick: choose a modifier that rewards a playstyle you already execute — Fragment volume for Fireworks Master, Growth scaling for One-Shot Nuke, or dual-status uptime for Dual Element. Avoid random +damage Pearls that assume perfect dodging if you still die to trash waves.

Second Pearl: add economy or tarot flexibility if your team can cover the new penalty in co-op. Assign healing penalties to the player running Mysterious Nun or defensive relic cores.

Third Pearl and beyond: endgame territory. Document combinations in community threads and cross-check updates after patches — Pearl numbers shift faster than base relic tables.

Co-op Pearl etiquette

Pearls apply to the lobby, not individual players silently opting out. Hosts should read each Pearl aloud before matchmaking. If one Pearl bans skill usage during Silence windows, the party needs non-skill DPS from weapons and affixes — not ability spam comps.

Trade relics toward whoever covers the Pearl penalty: healing Pearls go to support builds, economy Pearls to Money Whiz, break tools to One-Eyed Codger.

Pairing with Forge and Mythic farming
